M3: Where can I get a breakdown of labour and materials that make up the rates?

The labour and materials files (SMVs) are available for purchase to M3NHF Schedule of Rates subscribers. You can select them from the M3NHF Schedule of Rates menu on the order form. You can place an order there, or just get a quotation.
